    March 17,2002 at 11:10 a.m. I was assisting Chris Gould with the target scope alignments during on-call duty in Hall A. I was standing appox. 30 ft. away from the target chamber when I heard a boom from the target chamber area. I turned toward the target and saw a white plume of gas escaping from the upper taget chamber relief valve. There were pieces of super insulation material floating to the hall floor and settling around the target platform. I went to investigate what happened and noticed the target chamber was littered inside with super insulation and the
    port windows were fogged and cold to the touch. I immediatly called Mark Stevens
    to inform him of the situation I also called Dave Hammlett (on call tech.) for the radiation control group to respond to the possible release of contamination in Hall A.
    Mark Stevens instructed me to inform everyone in Hall A to evacuate the area.
    Mike Seeley, Chris Gould, and I left the Hall within 10 mins. of the incident.
    I asked Mike Seely to inform the MCC crew cheif of the situation in Hall A.
