I made an escorted access to check the missing T6 trigger (LHRS+E-plane).
First I thought there was a bad output on the coincidence module for the 
T3 (LHRS) trigger.  However when I had the coincidence module in "AND"
mode, I could not see a coincidence even though I could see T3 and E-plane
singles overlap on a scope.

Eventually I traced the problem to bent pin on a 1 ns cable where the T3
trigger is taken to form the coincidence between T3 and E-plane singles.

I replaced the cable with a 2 ns cable, since I did not have another 1 ns
cable handy.  This restored the T6 coincidence trigger.
